POSITION TITLE: REFERRALS PROGRAM ASSISTANT – Bilingual English/Spanish

FLSA Status: Non-Exempt

REPORTS TO: Referrals Manager

DEPARTMENT: Referrals

WORKSITE: Ravenswood campus

ORGANIZATION

The mission of Ravenswood Family Health Network (RFHN) is to improve the health of the community by providing culturally sensitive, integrated primary and preventative health care to all, regardless of ability to pay or immigration status, and collaborating with community partners to address the social determinants of health.

POSITION SUMMARY

The bilingual Referrals Program Assistant is responsible for the administrative functions of the Referrals department:  greeting visitors, confirming external patient appointments, acting as a resource agent for the organization, and creating word and excel documents.

DUTIES AND RESPONSIBILITIES

To be performed in accordance with RFHN Policies and Procedures

  1. Assist with the coordinator of referrals:
  • Process and track specialties referrals status.
  • Fill out various forms and applications as required for each patient.
  • Answer and help providers with tasks related to referrals. 
  • Fax forms to specialists and external organizations.
  • Scan clinical notes and other patient information into each patient file.
  • Follow up with various paperwork/external organizations regarding referrals.
  • If required, accompany patients to external clinical appointments.
  • Understand and give directions to patients on how to apply for all County Health HMO’s or 

County Health Programs.

  • Perform administrative functions as needed.
  1. Foster an environment that promotes trust and cooperation among patients and staff, maintaining confidentiality concerning patient and employee information.
  2. Attends staff meetings and RFHC organizational events, as requested.
  3. Other duties as assigned and requested.

QUALIFICATIONS

  • Completion of COVID-19 vaccine series and booster 
  • High School Diploma or equivalent (GED) required.
  • Bilingual Spanish/English proficiency required.
  • Experience working in a community health clinic preferred.
  • Basic knowledge of computers in a Windows environment.
  • Outstanding customer service abilities.
  • Ability to communicate effectively verbally and in writing.
  • Current Basic Life Support (BLS) certification.
