ORGANIZATION

POSITION SUMMARY

The primary responsibility of the Prescription Refill Nurse is to support the assigned Health Centers with the management of prescription refills, monitoring Emergency Department and Hospital Discharge Reports, and assistance with outside record audits.

DUTIES AND RESPONSIBILITIES

To be performed in accordance with RFHN Policies and Procedures

Prescription Refill Management: 

  1. Responsible for accurate entry of information from faxed refill requests into the EHR system and submission of the request to the appropriate clinicians.  Communicates with the pharmacies on behalf of the agency clinicians regarding prescriptions questions.
  2. Responsible for monitoring the Unmatched Refill Box in the EHR.  Information in the Unmatched Refill Box must be verified, corrected and submitted to the appropriate clinicians for approval.
  3. Responsible for monitoring the Fax Status box.  Trouble shoots each item to ensure the prescription is successfully sent to the designated pharmacy.
  4. Completes Medication Prior Authorization forms and Medication Treatment Authorization forms with correct information from the patient record and faxes or submits to the appropriate RFHN Provider for signature and submission.
  5. Submits the Medication Prior Authorization, Medication Treatment Authorization, and Durable Medical Equipment forms to the HIMS department for scanning into the EHR system and complete documentation in the patient record. 
  6. Tracks successful daily count of refills, prior authorizations, and durable medical equipment requests, and reports numbers to Supervisor weekly.
  7. Monitors refill requests for clinicians who are out of the office and redirects to appropriate covering clinicians.
  8. Ability to answer medication related phone calls, troubleshoot potential medication issues, and outreach pharmacies and other agencies when needed.

Clinical Information Monitoring and Management:

  1. Monitors for Emergency Department and Hospital Discharge reports daily.  Verifies the patient’s information and ensures a follow-up appointment is made with the appropriate Provider.
  2. Cultivates and supports primary care and subspecialty co-management with timely communication, inquiry, follow-up, and information integration into the care plan regarding transitions-into primary care and referral. 
  3. Assists in retrieving patient information from outside entities in preparation for the follow-up visit.
  4. Escalates patients with health issues that require further evaluation, additional services, including Social Service needs to the RN Care Coordinator.
  5. Establishes and maintains a relationship with ED staff/social worker to coordinate patient follow-up care with PCP.
  6. Identifies trends that prevent patients from receiving care after ED/hospital discharge.

QUALIFICATIONS

  • Completion of COVID-19 vaccine series and booster
  • High School Diploma or equivalent (GED) required.
  • Bilingual Spanish/English proficiency preferred
  • Associates Degree strongly preferred; graduate of accredited nursing program.
  • Must possess a current California RN or LVN license
  • Experience working in an electronic medical record environment highly desirable.
  • Ability to assume sole and independent responsibility for various assigned projects and tasks.
  • Ability to exercise discretion and independent judgment with respect to significant matters.
  • Strong insight into clinical processes.
  • Strong organizational, time management skills and attention to detail.
  • Strong and problem-solving skills with the ability to focus on detail.
  • Strong ability to identify potential issues and participate in their resolution. 
  • Ability to work on several projects and assignments simultaneously. 
  • Ability to memorize, retain and recall pertinent skills and information.
  • Proficient in the following Microsoft Office applications, including Outlook, Excel, and Word; proficiency with PowerPoint.

  • Able to work with protected health information and maintain absolute confidentiality and privacy in accordance with HIPAA regulations.
  • Ability to organize multiple tasks and manage competing priorities.
